IPAD and Android app development

We're after someone who can turn the attached file into an app that's ipad and adroid friendly. There'll be some modifications, but the pages give the general layout of the info captured. Here's the detailed info on what we require: - The option for each user to enter their own pre-filled answers (and then be able to select these answers via a drop down box) - Ability to change company logo on the pages - Auto fill the name of the person taking the application - Auto fill the name of the applicant(s) - Ability to sign the screen in the signature section - Automatic prompter to ensure all required fields are actioned - A final page summary of the information, this is then automatically saved on the ipad in pdf format and emailed to a pre-programmed email address when an internet connection becomes available Additional options: - Ability to integrate the information captured with 2 different CRM's we use (mercury and podium- mortgage application based software). This is a very simplified app that we are intending to be used within our own organisation, through our 2 different business names.
