354210 Form to Preview to PDF

Need a single page input form -- can be inserted via php include into our template. This is unlike our other forms because we do not want to store or collect the information. This is a tool for our members to generate a document for their own use. The document they will create is called a "Creative Brief" The form has 11 text fields (textbox limit size 400 characters). The user will fill in each of the fields, and then press generate. We will display the output on the screen, and they can press a button to generate a PDF or print the data. We do not want to retain the data. It is gone with the session. We have a form generator, but don't need captcha, store to database, etc, since we don't want the data. This is for them to play with.
