Simple Dreamweaver Form FIx

We have a product website ([url removed, login to view]) that has a submission form that is not working. We are currently swamped and need it fixed ASAP (I.e. today). Basically the form with all of its contents needs to be sent to rwille at vandamme.com. Probably a 5-15 minute job for a good Dreamweaver person.

## Deliverables

1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Installation package that will install the software (in ready-to-run condition) on the platform(s) specified in this bid request. 3) Complete ownership and distribution copyrights to all work purchased.
