Fix site built on here but not 100% checked for function

I had my site [login to view URL] built by a designer on here. They never checked to make sure the "contact" function was working. 28 days later it still does not work. Also when the template update came out and I updated it destroyed my site. ALL the content is there, I need a header fixed and replaced and also the contact page plug in to work. Files attached that are the header and logo. This is a minor job and can be done in a hour or less. I just need help and this to work prior to building two more sites.
