Modify Wordpress theme and add new function

Hi i have this website: [login to view URL] its a directory of doctors, if you can see i have 2 types of profiles, 1 is the featured profile (blue button) and the normal profile (grey button). I need add one type of profile more, i attached an image for you and please see how need the new type of profile, in one line, and this type of profile only shows at the end of the results. (I need that shows first the blue profiles, second the grey profiles and at last this new type of profile) The internal profile i need the same as the another types of profiles, with images, contact form, map, etc. only change the type of display in results. this new profile i need that dont allow show in featured areas of sliders or the doctors in the homepage. And the second change that need for my website is create a pagination when you search. only shows 20 results per page. please see the example in the image that i attached here. Thanks.
