Socialenigne button on profile and contact form

Hi, I have a website (socialengine4) which has two profile-types one for pupils one for teachers. I need a button-widget that I can add via the layout-editor (drag on one profile type - see picture 1). The widget is simply just a html button that when pressed leads to a copy of the internal contact form. (this should be simple by copying the existing internal contact form and just modifying it). After button-click -> contact form with the following fields (!) : besides the standard fields: email from: [field of the user ( in this case the pupil) - taken from the database] email to: [the teachers email address - taken from his profile] Name: [pupil name - from profile] First-name: [pupil first-name - from profile] title: [textfield] interested in (degree type):[textfield], disciplines[textfield],subjects[textfield] educational background (courses & results): [textfield] IELTS score: [textfield] employment & qualifications: [textfield] full-time or part-time:[textfield] comments:[textfield] contact number: [textfield] email:[again pupil email from profile] submit button (Standard) The idea is that pupils will browse teacher profiles and then click on the button which is only on the teachers profile. So on the teachers profile there is the apply button and it leads to this customised and copied contact form. Pupils can basically send an email to the teachers by using this. I only need the working widget contact form to install ( incl. documentation or installation instructions) - no need to create profiles etc only the button widget and contact form.
