Good morning. I can create the form in PHP that will save the data straight to the SQL server. You don't need to open or forward any port through firewall for SQL Server if the web server resides on the same server as SQL Server. Otherwise, if the web server is on a different machine than the SQL Server, but within the same LAN, still a good network administration would allow the PHP to connect to the SQL Server without a trouble considering the connection private in the same LAN. Otherwise, you need to open TCP 1433/1434 in your router that would forward all the traffic to the SQL Server machine through that port. Please let me know if you find me helpful in this regard. - Joy
