Pls copy this appointment form for my html5 website: [login to view URL] At the end it should send an email with all data to my email-adress. First job is to copy the form, then we will need some small changements so that i can use it. Form needs to be responsive Sundays, saturdays, past days and the current day should be dissabled. Exactly same design then example I need six different appointments each day, so 6 diferent possible times that the visitor can choose. Appointments that are already booked should be dissabled. Days that dont have any appointments left should be disabled.
