Building a website with a complex form

I would like to create a website with a sign-up form. So far, I do not have anything. As such, I need somebody to recommend the best approach. The website is just one page, with a video integrated, a bit of text, and some design elements. I will provide the video, text, and I have a designer. The most important part of the website, however, is the sign-up form. The sign-up form consists of six sections. When you initially go onto the website, there is a little form floating that asks a few questions. Once this form ("section one") is completed, the person clicks “Submit” and section one disappears, giving place to section two. Visually, this change from section one to section two should make it look like there is no change of page - section one zooms off to the left, and section two replaces it, zooming in from the right. Everything else appears to stay the same. This happens again from section two to section three, three to four, four to five, and then finally five to six. There is a progress bar at the bottom of the form, indicating which section the user is on, out of six. Section one has three questions: first name (user submits own text), last name (user submits own text), year of graduation (user selects from drop-down list years 1950 to 2019, in reverse order so that 2019 is at the top, then scrolling down goes to 1950). There is a submit button that leads to section two. Section two asks one question: "Which city is closest to where you live?". There is a list of little pictures with text underneath each picture. Each picture (plus the respective text) represents a selection (in effect, a city). The user selects one option and presses a submit button that leads to section three. Section three is similar to section two. It asks one question: "What college were you in?". There is a list of little pictures with text underneath each picture. Each picture (plus the respective text) represents a selection (in effect, a college). The user selects one option and presses a submit button that leads to section four. Section four is similar to sections two and three; however, the user can select multiple options. It asks one question: "What courses did you do?". There is a list of little pictures with text underneath each picture. Each picture (plus the respective text) represents a selection (in effect, a course). The user selects as many options as they wish and presses a submit button that leads to section five. Section five is similar to section four. It asks one question: "What sports did you do?". There is a list of little pictures with text underneath each picture. Each picture (plus the respective text) represents a selection (in effect, a sport). The user selects as many options as they wish and presses a submit button that leads to section six. Section six is a little text that thanks the user for submitting the form. All this data should be transferred to some form of spreadsheet or data processing method where, on Row 1, Column A is first name (section one), Column B is last name (section one), Column C is year of graduation (section one), Column D is the city (section two), Column E is the college (section three), Column F is all the courses (section four), and Column G is all the sports (section five). Note: PHP with MySQL for data processing is perhaps the best option, but I am open to any suggestions. When bidding on this project, please outline what approach you will use.
