Create an Online Registration Form with Calculation (Radio Button)

Create an Online Registration Form (2 type) the website was ready.. only the registration form needed. here is the scenario… Type 1 1) on the registration form, it will start at 'Title' until 'Are You a Vegetarian'… this just a normal form and need to be stored into admin email/spreadsheet (e.g. google docs) 2) 'Registration fee' section, the applicant need to tick either one of those part (will use radio button) all of this have a costs. eg. check the attachment. - International early bird / regular / 21 july - local early bird/regular/21july - student (international) early bird/regular/21july - student (local) early bird/regular/21july - cultural activities early bird/regular/21july - hotel early bird/regular/21julyearly bird/regular/21july - dinner early bird/regular/21july (so.. this part… the total of amount must be displayed as 'total fees' before applicant click the submit button… which they can see and make an amendment if needed) after all entries are confirm… applicant will click submit button… and the word 'thank you and bla bla blaaaa will appear at the screen. and the data will be email or stored in the spreadsheet (google docs). 3) 'Payment made by' Section. just use radio button and no need to calculate the items. Type 2 finally, when applicant made a payment. he/she will have to upload the receipt (image or pdf) from the form and their file will be send to admin email as attachment. both the links for this online form need to be put on the weebly website (will be done by me and not included on this project. the form also can be edited by me specially the content or fee amount. so.. if you can do with google docs or any free and easy tools.. just suggest okay?
