Questionaire

A system to be able to ask/present up to 10 Multiple Choice Pre-questions to a web user before they are redirected to a different webpage in a preexisting website. These questions should be able to be created by an Admin and the questions and answers including the user’s responses should be stored in a database. There should also be functionality available to create one Report to view all questions and answers and responses for a particular program. This report should also have the functionality to export the data to an Excel File or file formatted to be imported into an Excel Spreadsheet. There is NO user sign-in or Registration, or passing or failing grade required as this as this is NOT a test system, but only up to 10 possible Pre-Questions asked/presented to web users before allowing them access (redirected) to another web page on the website. It would be a page with questions that the user would have to answer before moving any further into the site. The particular page it’s redirected to would also be setup in the Admin Section. Project Clarifications (Scope of Work): 1. In an Admin Section/Panel; need to provide functionality where an Admin can create a set of multiple choice questions and answers numbered anywhere from 1 to 10 which relates to a particular Program Title and be able to publish those questions to be used as an entry to a preexisting page/Program page on a preexisting website. 2. One set of these Pre Questions will be related to a Single Program at a time. 3. Questions will be Multiple Choice Questions w/ Answers 4. Admin will be able to add, edit or delete the multiple choice questions and answers once created. 5. There will be NO pass or fail criteria as this is NOT a testing system. 6. Questions once created will get listed in against the Program it’s been related to. 7. User needs to answer these questions before they will be able to navigate to the other section / Program page of the website. When a user comes to the website, he will/could see several Program titles on a landing page that would be clickable. Once the user clicks on a Program, they will land on that particular Main Program page. Here there will be other preexisting Program related content provided and developed by Customer. There will be a button/link which might say “View” or “Continue”. Once user clicks on “View” or “Continue” button/link, that’s when these set of questions, which Admin has created, will/might pop up a stationery modal window or user might be taken to another page with these Pre Questions. Once this Pre Question Popup Window is launched, or they have landed on a Pre Question page, the user will NOT be able to go any further on the Website (taken to the Program) until they answer and Submit these questions and answers or they Click on an available Cancel Button or Link to go back to the previous Program Content page. Once the user answers all the Pre Questions and clicks on Submit, they will then be able to navigate (be redirected) to the other page (The Program) on website. 8. In the Admin section there will also be available access to generate one Report to view all questions and answers for any one particular Program and/or ALL available Programs to date. Admin will also be able to export any of these Reports (programs, questions and answers to an Excel spreadsheet import formatted File.
