Simple web app to record voice notes & create Asana tasks

I use Asana to assign tasks to team members. When I'm on the go, I've sometimes found it easier to record voice notes with instructions to a task and share these in a new Asana task. The process involves recording a note, uploading it to the cloud, creating a new Asana task and appending a link to the voice note. I would like to create a one-page web app to streamline this process. I suggest we achieve this as a web-app to allow it to be easily used on both mobile and desktop scenarios. The app will be very simple in its UI/flow. There will be a record button; once pressed it will begin recording sound via the browser (using HTML5 rather than Flash). The user presses the same button again to stop recording (or a second button is fine). The user could then have the option to listen to a playback, or submit. Upon submission, the file will then be saved to a location and a new Asana task created via the API. The user should have the ability to re-record also – perhaps by pressing record again, or there being a cancel/clear button to return the user to a fresh instance of the app. The task should be automatically assigned to a particular user ("Dan Moredo"). The title should be "Voice task: 2015-10-31, 11:15AM". Timezone will be UTC+7. Should automatically apply the tag "Voicenote". There are a few options for where we could save the note. I suggest either Amazon S3, or uploading it direct to Asana with the task. The app can perhaps confirm that the task has been successfully created, and perhaps provide a link to the task URL. The app is very light in UI elements, but we can still use the Flat UI library for a clean look. The design/layout should be mobile-first focused; so buttons that fill 100% browser width with a high resolution. Open to further input on improvements to this workflow.
