I want to be able to have an app where members can send photos and fill out a basic form details from mobile devices, which can then be downloaded/printed by management. Something that can store information into a database so collated information can be stored, sorted and collated. Basically, it is a reporting process that allows photos to be attached to a basic form template, and then have that information able to be stored into a user friendly database and spreadsheet.
