161661 Form Layout & Design

This form needs to be designed better than I can do it. 1. Subscribe/Unsubscribe fields should be hidden 2. Maybe a very light color in form 3. Submit text should be 'Send Calendar' 4. Perhaps rounded corners on form area It is important that field names stay intact as this works with another program.
