02 - Build Custom Website with Questionnaire

We would like to create a website that will enable people in South Africa to answer a number of questions and then to dynamically create a Last Will & Testament for them. They can then see a draft and if they want it they make payment. The project will be done in 2 phases: Phase 1 - Create website with questionnaire. Users must be able to register, go back and update their answers. Also have a vault to upload supporting documents. All data to be saved to a database. We have used Google Forms to create the questionnaire and this will serve as the specification. See [login to view URL] The text and flow as in the above forms must be used as is and not be changed in the web site that is to be created. The client must also be able to see the site in another language, Afrikaans. Google translate must not be used. The functionality must be created to switch between Afrikaans and English. Once the site is built in English, we will provide the Afrikaans words & sentences for all the pages. Phase 2 - Use the data collected and create a .pdf file using the logic and template we will provide - a sample (Single Will - Questionnaire (Completed).pdf) is available now but the final template must still be created. Create a payment page where users can buy a membership upon which they have free access to the Will and they are allowed to make changes to the questions and create a new Will. More phases will follow after that, for example Phase 3 will allow the client to capture his assets & liabilities (his personal inventory). A separate specification will be created for this.
