I need a form

We have a website to provide trust seals and guarantee a website credit for their customer. we need a signup form for our member to be filled include their details, their information, their company information, what they are doing and all details they can put in a 5-6 pages form. I need a professional design. The form details is not going to provide by us, you should find them all, but we can lead you to Banks form or when you want to get a loan from a bank what they are going to ask you. As it is not a normal job we can only pay 10% in advance.
