iPhone application : contact form + dynamic multiple photo upload feature

features needed : a contact form + photo upload feature. I need a simple app that will allow a user to submit his contact info to a server that I provide (preferably json): 10-12 fields. then a second page will allow to take photos from the camera or the gallery. the number of photos must be dynamic. each photo taken will be inserted in a grid. (see the attached image) there must be a minimum number of photos (2) that are mandatory but the user can then add any number of additional photos (with a hardcoded max number). each photo can be reviewed (magnify icon) or erased(cross icon). After a form validation , the prog must upload the form data along with the photos to a server that already exists & has a REST api. a progress indicator must be displayed during the upload. Last: You will help me deploy / publish on Apple Store the final app
