I am seeking to create a pdf or MSWORD form (PDF preferred) from the text on this attached image. Instructions and notes:
*Form to include field input boxes, drop down menus, and check-boxes that can be edited by end user. All other text should be static and unchangeable.
*When the document is saved it should save with the input made by end user.
*Two columns per page.
*Keep the layout the same. Ie If there are three boxes in one row on the image, the same should show up on the pdf form.
*Page break location is not critical; however, page break should not be in the middle of a "rule/section". (Ie "EMA General Settings" is a section)
*Deliverable will be two documents. (1) One will be the native file so that the form can be easily edited by another person in the future. (2) the second will be the end user document ready for the end user to fill in. [If the two desired documents are the same, send a brief explanation on how to edit the form.]
*COLORED highlight and text is a Color code for freelancer to create the form.
-Yellow: All of these drop down menus will be the same. There will be three options from which to choose: FAST, MID, SLOW.
-Blue: These drop down menus will be very similar to the yellow highlight. They will have one added unique option.
B1:FAST, MID, SLOW, DISABLED
B2:FAST EMA, MID EMA, SLOW EMA, PRICE
B3:FAST, MID, SLOW, MIN
-Red: These are all unique unrelated drop downs.
R1:LinReg, Simple Slope
R2:None, % of Asset Price, Ratio to ATR
R3:EMA-X buy-ins, All buy-ins
R4:EMA-X buy-ins, All buy-ins
