Call Tracking Webbased with MYSQL

Develope a webbased way to track our call listing Pages Needed ASP or PHP with Mysql datebase backend. Must run on a Windows Server. Login Page Username and Password. Call List Page Next Button Auto rotate base on date or when tried to reenter. Name must not appear more then once is a 30 day period. Client Id Call Type Must not show certain type First, Last Names Office Number Cell Number Fax Number Email Date Last Call Who Last Call Auto Enter from User Lead drop down from Username Area Notes with timeline off previous notes. Call List Search page by choosing field and text box. Admin Area Add/delete new user Username Password First Name Last Name Position Add/delete position Add/delete location Add/delete Call Type Add new client with a cut and paste format , excel, and tab file upload. Need to verify not in database and if in database not to add then push to top.
