Data Capture & Personalization Script

Hi, I'm looking for a script that I can use to capture data about the visitor through a simple survey form on an index page . The captured data would then be used to personalize a series of messages in an autoresponder sequence and would also be used to personalize the text and the images on the subsequent page that they visit. The script will include: An Admin Panel with the ability to: 1. Set field titles for form data 2. Configure field response type (text area/radio button/check box) 3. Input Autoresponder details including URL, email, name, custom fields. 4. Input landing page URL and custom field/includes data. 5. Configure the data capture page with editable templates (optional) 6. Configure the personalized page with editable templates (optional) 7. Review responses by type from collected data. The script would output either: a) HTML/PHP code to insert into data capture page or optionaly... b) HTML/PHP code for the entire page to allow editing in WYSIWYG Editor. The script wold also post the captured data to a MySQL database to allow for reporting. Please Note: 1. The script must be fully commented and documented and muct include installation instruction documentation. 2. The script must not include any open source or GNU code. ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables): a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment. b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request. 3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement). ## Platform PHP/MySQL
