Improve 4 order pages for e-commerce site

This is a straightforward task for someone with html, php and css skills. I have 4 existing php order form pages for different products that need to be styled in a very specific way. 3 of these will be designed in the same way. I have an existing template file I want you to replicate for these 3 pages. All copy etc will be provided. The fourth page is slightly different. Again, I have an exact template I would like you to replicate, but this will include some coding e.g to change the display on the page if a checkbox is ticked or not, and to hide part of the form from view. Nothing complicated for anyone with experience. I need this project to be completed ASAP so please only submit a bid if you are available to complete this work within 48 hours of project start.
