Web-App for Survey

The project involves building a webapp for conducting a specific survey online. The webapp will be built/hosted on the LAMP platform. It will be required to fit into the theme (look/feel) our current website ([login to view URL]) The webapp is required to plug into the vBulletin DB to get the usernames and keep it in sync. The basic function of the webapp is to feed a randomized set of questions to each user and record their answers in a DB. The webapp should also be capable of generating reports on the answers selected by the users. Detailed specs can be given on receiving competent bids. The project is needed on a rush basis. We have about a week or 10 days at the most.
