Form Wizard and Validation

Development of a standard form wizard that steps the user through filling out a single form in 5 steps with submit to database as the last step in the wizard. Needs to have client side form validation (javascript or parsley) for required fields and ensuring proper format for phone number, dates, max character size limits, etc. User can select next step or previous step as they fill out form. The first step will need to have a ui autocomplete function where as the user types the customer's last name, any matching customer names in the database will be displayed in the drop down under the form whereby the user can select and have all current customer fields autofilled. If the customer last name does not have a match, the user completes the form for a new user. The rest of the form fields are very standard and should be no issue, easy project for the right developer. Please let me know if you can complete this task.
