Email Response to web Form

Modify an existing live website to automatically send an email to both the sponsor of the site and to the person "enrolling". The actual dialog is attached. Set up a page that will allow the "sponsor" of the site to email all of his/her people who "enrolled" on his site and the other enrollees who land in his/her group.
