Database in the cloud, or network, that manages request services by form entry and excel sheet upload

We need to integrate the orders request made by phone and email. We make clinical tests on-site, so we received orders from companies and individuals. Usually, we enter the phone request in an electronic form (googleform) and email orders by excel sheet (with this list patients and their addresses). We need a system, in cloud, or network, that allows entering all the data, by Form and uploading excel sheet. Generate an ID of each service, with two tables ( one for companies, another for all the patients(include individuals)), and get customized datasheet output. The ideas generate 3 automatic query output with the option to change the fields included(nothing so complex, only selects some fields for each one) (we need one for the route optimization process, another for patients systems, so on). we are not sure if it is better in-network or clouds, we want explorer both alternative (We have a hosting that can we use to host the system in the cloud or a local server). I attached templates with all the fields (in Spanish), many are repeated (all are simples field, with some Combobox).
