Online form Automation Project

Our project consist of taking form [login to view URL] and making a web based template of this form for the nurses aids to fill out and print at the end of the week for client to sign. We want the data base backend to be MySQL. The form is to be written in PHP. There should be a user table and a form table to store the data from each form that is filled out each week. This form also doubles as the time sheet for the CNA. The hours at the bottom of the page must be tallied by the form but the total not changable by CNA. There will be a new record for each patient each week that the CNA is caring for that patient. The form data must be keyed to the CNA. There must be a reports function that allows admin to retrieve the CNA's form for the week. The database will also archive the form data. Data retrieved must populate the online form for online viewing of each patients record that is stored. The CNA's must log in to create, view, modify and submit completed forms. Only admin will be allowed to run reports. One of the reports should only display time totals for the CNA's for the week rounded to the nearest 5 minutes.
