Resolve problem with email form pulling address from database

My web application is built on ZEND and allows me to create a sub-domain with unique user accounts stored in MySQL. Each user account has unique logo, phone number, contact info and email address and other info. There are also ADMIN accounts created in MySQL. On each sub-domain uses the same template form to request a quote for life insurance. This form should be sent to the email address in the MySQL database for the sub-domain account and CC'd to all ADMIN users. THE PROBLEM: Currently all emails are sending to a single email address. I need this fixed so that each sub-domain form sends to the sub-domain account email as specified in MySQL and CC'd to all ADMIN users. NOTE: this was working before the last round of changes where the templates used for the subdomains were udpated.
