Survey website with DB and more detail

PLEASE READ THE DESCRIPTION CAREFULLY ********************************************** Description about the website: The main idea of the website is to create a survey with ONLY 3 options. Only who has an account can create a survey. To create the survey, user has two options: First Select 3 options from list. (You can make it empty, we are going to write it later on) Second to write 3 options ONLY. The title of the survey is always "Chose one of these Option". After creating the user can post it to public to vote on it. The public can only vote once (BY IP ADDRESS). The survey can published ONLY 24H, after that the user can see the result (Percent and number of responses for each options). The result should be private only the user who created the survey can see it. After 24h no one can vote on the survey, and the survey moved to history. The user can only delete the survey, he/she can not edit it. After each survey, the most option voting can be saved in a list in DB. So admin can see it. HTML5: Simple as possible for user, easy to understand and use. And it is possible to edit it in the future. We are to translate it to another language later, so just add an option to switch between these two languages. (EN/AR) CSS: Very good design and respon website. (Works as it should be on Mobile and Desktop) PHP: Possible to edit in the future. Sign Up: Sign up form should contain [First Name, Last Name, Username (unique), Password, Email (unique), Age], user can register only by (Gmail, Hotmail, Outlook and Yahoo). Add Facebook button to Sign Up by Facebook. User should confirm the email after first login. Add check box to confirm that user is over 15 years Old. ِAnd captcha code. Login: Login form As usual. After login, user can edit his/her profile setting, add his/her photo and small box to write a bio. Note: I will connect this website with Google Adsense, so think about place for ads.
