This project generally requires the development of three (3) distinct features of a web-based service: 1. Web forms for the submission and editing of data – this shall be used at the back-end of the online service wherein enumerators/data entry officers can easily post textual data with the following fields: a) Nine (9) ‘external factors’ (ideally a dropdown menu) b) Sub-title field (also the subject field) c) Text area field that can contain up to 5,000 characters and must have paragraph editing options, numbering options, and bulleting options. d) Automatic date and time stamp to show when the textual data is uploaded/posted e) Automatic name and surname stamp of enumerator/data entry officer who posted the textual data. 2. Retrieval of such data from MySQL database for posting on a separate web page – at the front-end, all data that were posted on the database must be reflected on a separate webpage. Each ‘external factor’ must have its own webpage and all sub-titles must be presented as list of hyperlinks. When clicked, the webpage associated with the hyperlink must be able to: a) Show the ‘external factor’ as main title on the upper left part of the screen (on Heading 1), b) The sub-title (12pts, bold) below the main title, c) The posted textual data from the text area (10 pts, regular) that should be below the sub-title portion and with the proper paragraph formatting, d) The date and time stamp (10 pts, regular) on the upper right corner of the screen, and e) The name and surname of the enumerator/data entry officer who posted the updates on the upper right corner of the screen below the date and time stamp (10 pts, regular). CSS shall be applied for accordingly with 'Arial' as font type. 3. Being able to download those same data from the web page as a pdf file – at the front-end, after viewing each ‘external factor’ update, each corresponding web page must have the pdf printing function. This means that all contents of that webpage (main title, sub-title, textual data, date and time stamp, name stamp) can be viewed or printed as pdf on a letter-sized paper with portrait orientation. The marginal notes should include the date and time the pdf file was generated, the main title and sub-title.
